9. Re: Corporal Punishment forms signed by both adooptive parents
10. Evacuation Plan
11. Verification of Smoke Detector completed by home study worker
12. State Police fingerprint cards completed and sent to ISP for state clearance
13. Photocopies of birth certificates for each household member
14. Photocopy of marriage license and divorce decrees, if applicable
15. Photocopies of local police clearance letters (originals needed for dossier)
16. Pet inoculation certificates
17. Well water report, if applicable

Fees
Fees include: application fee ($250), home study fee ($2050), parenting class ($125), post placement ($800). Once the child comes to the U.S., should you choose to readopt, there is a court report fee ($150). Readopting in the U.S. is required if you choose not to see the child prior to the completion of the adoption.
These fees cover the interviews, preparation of home study, the parenting class, and five post placement visits.
